Program Overview
The Continuity of Care Pharmacy Initiative is a Medicare and Medicaid initiative designed to recognize and reward independent community pharmacies for their efforts in improving medication adherence, fostering stronger patient relationships, and supporting the continuity of care.
Through this program, pharmacies earn a monthly payment for each patient enrolled by helping them remain consistent with their medication regimens using a Remote Therapeutic Monitoring (RTM) adherence program. Successful pharmacies can earn over $50,000 a year with minimal time spent per patient as BettrAi’s in-house telehealth team provides the actual monitoring and clinical support services along with the billing of the CPT codes.
