From FLAACOs to the Frontlines: Key Insights for Value-Based Care Visionaries
Just over a week ago, I had the privilege of attending the Florida Association of ACOs (FLAACOs) annual fall conference. It was my first time attending as CEO of BettrAi, and the experience left a profound impression. The energy in the room was palpable—a gathering of leaders, innovators, and changemakers all working toward a common goal: advancing value-based care.
For me, four key themes stood out, and they’ve reshaped the way I think about where we’re headed—not just as a company, but as a partner in this collective journey toward better, smarter, more value-driven care.
1. Long-Term Care’s Shift to Value-Based Care: The Time Is Now
The movement of Long-Term Care (LTC) to value-based care (VBC) is no longer a distant possibility; it’s here. What stood out to me most during the conference was the recognition of LTC as a critical player in achieving the Quad Aim—improved patient outcomes, better population health, reduced costs, and enhanced provider satisfaction.
However, there’s a glaring gap that can’t be ignored: fewer than 10% of LTC facilities currently participate in an ACO. This highlights a massive untapped opportunity for LTC providers to embrace value-based models and align their care strategies with the goals of ACOs. The benefits are clear—enhanced care coordination, reduced costs, and improved outcomes—but the path forward requires the right tools, strategies, and support.

3. Most LTC Patients Are at Home: A Call for Connected Care
Here’s the statistic that stuck with me: fewer than 30% of patients receiving long-term care are in a facility. The majority—over 70%—are at home. This reality demands a shift in how we approach LTC. We need systems designed to support patients not in traditional care settings but in their own environments.
This isn’t just a logistical challenge; it’s an opportunity. Patients in home-based care settings often face unique barriers, but with the right tools—such as remote monitoring and predictive analytics—we can create a safety net that ensures no one falls through the cracks. Think about it—patients can offer invaluable insights into their daily lives, from medication adherence to symptom tracking to lifestyle choices. Incorporating this data into care plans could revolutionize how we approach compliance, creating a more holistic view of the patient’s health.
